Print and complete this application form to enroll a camper at Mishawaka. Instructions for mailing and payment are included on the form. - General Information
Read for some quick answers to frequently asked questions about the summer camp experience. - Parent Handbook
Includes all the information parents need to know. This is the same document that is mailed to you in a booklet form after we receive your registration. Includes information on mailing address, health care, packing list, transportation, as well as lots of other general camp information. This document is 200kb and may take a little while to download, especially using a dial-up connection. - Packing List
This will help guide you in the packing process. Remember, camp is an outdoor experience so don't pack things that can't get dirty.
The following forms should be printed, filled out, and faxed or mailed to Mishawaka before the start of the summer session. Our fax number is 218-326-9228.
- Pre-camp Questionnaire
This questionnaire gives us an opportunity to learn about our campers before they head off to camp. - Health Form
This form needs to be filled out and mailed in prior to the camper’s arrival. Please keep a completed copy for your own records. - Asthma Form
If your camper takes special steps to manage asthma, please fill out this form in addition to the health form. - Charge Authorization Sheet
This form authorizes specific camper charges and spending. - Trip Permission Form
This form gives permission for your camper to attend various extended trips. - Rafting Trip Release
Superior Whitewater, the outfitter that runs the rafting trip, requires that their wavier be signed by a guardian of each camper wishing to participate. This link will take you to a pdf version of the waiver on Superior Whitewater's website. You can download, print, sign, and send or fax the waiver to Camp Mishawaka. - Horseback Riding Permission Form
This form gives permission for your camper to participate in our horseback riding program.
